How the claims process works

  1. Download and complete the applicable forms below
  2. Submit the appropriate documents by attaching them to an email and emailing us at support@imresilient.co
  3. Please include a phone number where you can be reached. You will receive a call within 24 hours from a representative to review your claim and ensure your documents are in order
  4. The claim will then be submitted to the insurance company for final review and approved claims paid out within 15 business days

Which forms do I need?

To claim your Life Insurance benefits as the named beneficiary please provide:
  1. Completed claimant statement
  2. Original (or copy of the original) Death Certificate
To claim your AD&D benefits as the named beneficiary please provide:
  1. Completed claimant statement
  2. Accident report
  3. If the accident resulted in death, please also include
    1. Original (or copy of the original) Death Certificate stating the death was an accident
    2. Autopsy report
    3. Toxicology report
To claim your Life Insurance or AD&D benefits if there is no named beneficiary, please also include the:
  1. Survivor / Estate Affidavit
